ReplyDeleteMe and Hesham formulated the following ttt plan for this case:
single phase ttt with fixed appliances
1. correction of crossbite of upper left 6 using TPA
2. extraction of upper right 8
3. Distalization of upper right 6 and 7 using miniscrew assisted mechanics to get molar class I on this side and regain space of extracted 5.
4. extraction of upper left 4, lower right and left 4s
5. segmented retraction for upper and lower canines and achieving canine class I (we will need maximum anchorage, using T loop in beta position and increasing beta moment)
6. bonding upper and lower incisors, alignment and leveling (use low profile brackets, MBT presciption). brackets for upper laterals need to be turned upside down for torque control
7. finishing
8. retention

ReplyDelete1. you need to refer the patient to a perio to evaluate before start of treatment
2. as we discussed before we need a problem list, objectives, appliance plan with time framework.
3. you guys went for minimplants to distalize the molar, yet you didn’t utilize them in your maximum anchorage situation
so, your treatment plan is absolutely amazing, it just need more details.

1- skeletal class 3 due to max retrusion
2-vertical growth pattern
3-proteuded and proclined upper and lower incisors
4-bolton descripancy
5-upper left 6 in cross bite
6-upper laterals in cross bite
7-class II molar relation on the right side
8-mid-line shift
9-crowding of upper and lower anteriors
Objectives:
1-correction of molar cross bite on the left side
2-correction of molar relation on the right side
3-correction of laterals in cross bite
4-correction of both upper and lower incisor angulation
5-correction of midline shift
6-relief of crowding in both arches
Treatment plan and procedure :
-single phase orthodontic treatment with fixed appliance therapy
-TPA to correct molar cross bite on the left side
-extraction of 8 and distalization of 6 and 7 to class I molar relation on the right side
-nance appliance for retention and anchorage
-extraction of upper left 4 and lower right and left 4s
-maximam anchorage ( group A)
-bonding upper and lower arches while dropping upper laterals
-canine retraction
-bonding upper laterals and their alignment
-correction of mid-line shift
-torquing laterals
-finishing
Appliance used :
Bands on molars and MBT slot 0.22 brackets.
